Hourly Care

Promise Health Care Nursing Services NJ Inc. provides hourly care services for clients who need assistance for several hours a day. Our certified home health aides (CHHA) are the ideal solution for our clients requesting hourly or respite care.

Our CHHAs assist with the following activities:

  • Tasks of Daily Living (bathing, grooming, incontinence care, meal preparation, laundry, minor housekeeping, etc.)

  • Diet and Nutritional Monitoring

  • Medication Reminders

  • Prescribed Exercises

  • Hospital Bedside Comfort

Live-In Care

The same services our CHHAs provide on an hourly and respite basis are also available to our clients requesting live-in care. Live-in care is a flexible, cost-saving option that provides round-the-clock safety, support, and peace of mind.

Nursing Assessments and Visits

Our visiting nurses (also known as home care nurses) interact with patients in their homes to render nursing care services that include medication administration, treatment, and other nursing interventions. One of the primary responsibilities of our visiting nurses is the patient assessment.

Promise Health Care Nursing Service NJ Inc. assigns qualified nurses who match our patients’ care needs and preferences. This way, our clients receive the quality care they deserve.

Once we receive the relevant script and referral by fax, phone, or email from a Case Manager, Social Worker, or the Individual them self, we will execute a RN assessment between 24-48 hours and commence full care afterwards. We are also equipped to handle same day emergency requests.

Companion Care

As our loved ones age, they often experience loneliness and difficulty keeping up with chores around the house. Sometimes all they need is a compassionate caregiver to visit with them regularly to engage them in conversation. Sometimes they need help with doing housework, meal preparation, or grocery shopping. Care Management

Care Management Services can greatly improve health outcomes, alleviate stress and increase function. Care Management is an essential component in navigating health care systems. When patients with complicated medical, functional and cognitive conditions receive care coordination through one of our highly trained Care Managers, hospitalizations and emergency room admissions are substantially reduced. Improved outcomes also benefit the support system around individuals receiving care, providing family members with greater peace of mind knowing their loved ones have professional advocacy when they need it most.

Our Care Management services are individually tailored to each person's unique situation and levels of need. We begin with an individualized assessment of a person's needs and goals, and then we create a Care Plan, which is like a road map, whereby we address all of the issues that were noted in the assessment, and we work to bring solutions and positive outcomes.

24-Hour Care

This service is similar to our Live-In Care services, with the addition of two or more caregivers rotating shifts to ensure someone is always awake and available day and night to help the client when needed, at any time. Elderly care can be difficult to provide for your aging loved ones, especially when you work, live far away, and/or have children that require care and attention. As their needs and demands increase, you can’t always offer the 24/7 care they require at home. You may try to schedule regular visits from caregivers and family members to attend to their needs. However, a decline in physical and/or cognitive functions or a change in a situation where one parent now lives alone may prompt you to consider round-the-clock care.

Moving your elderly parent to an assisted living facility or a retirement home is not your only option. Live-in and 24-hour home care can ensure a caregiver is available to address any personal needs – and provide senior companion care – day and night in their own home.
